Inspectors visited the restaurant on September 16.

They found insects and a urinal near the food storage area.

They also found meat blood residue in a refrigerator and food being handled on the floor.

The inspection reported problems with water, cleaning and food preparation.

Officials also said vegetarian and non-vegetarian foods were not properly separated.
