Officials found 3,400 voter revision forms at a photocopy shop in Kharghar.

The forms reportedly came from 11 polling stations.

Five booth-level officers, who help manage polling stations, were suspended.

Police also registered a case against a man named Suraj Patil.

Members of the Shiv Sena faction led by Uddhav Balasaheb Thackeray said they confronted him while forms were being copied.

A video of the confrontation spread online and led to an inquiry.

A BJP leader said the party was not connected to the incident.
