Inclisiran lowered LDL cholesterol more than bempedoic acid in a trial
A trial compared two medicines for lowering LDL cholesterol in 402 adults with atherosclerotic cardiovascular disease and high or very high risk.
The participants were randomly assigned to inclisiran or bempedoic acid, and all also received the highest statin dose they could tolerate.
Inclisiran was injected at the start of the study and again after 90 days, while bempedoic acid was taken once a day.
After 150 days, LDL cholesterol had fallen by an average of 56.31% with inclisiran and 15.35% with bempedoic acid.
The study said 78.2% of the inclisiran group reached their LDL target, compared with 20.3% of the bempedoic acid group.
Overall rates of unwanted effects were similar, though muscle-related events were reported more often with bempedoic acid.
The findings were presented at a European Society of Cardiology congress in Munich on August 31, 2026, and highlight the difference between the treatments for people at high cardiovascular risk.
A trial found that inclisiran reduced LDL cholesterol more than bempedoic acid in adults with atherosclerotic cardiovascular disease and high or very high risk.
After 150 days, LDL cholesterol fell by an average of 56.31% with inclisiran and 15.35% with bempedoic acid.
The LDL-C target was reached by 78.2% of the inclisiran group and 20.3% of the bempedoic acid group.
The Victorion-Challenge study included 402 adults and was the first randomized, open-label phase IV trial to compare the two drugs directly.
Overall rates of adverse events were similar in the two groups.
- Who
- 402 adults with atherosclerotic cardiovascular disease and high or very high risk took part.
- What
- A trial compared inclisiran with bempedoic acid for lowering LDL cholesterol.
- When
- The primary outcome was measured after 150 days. Results were presented on August 31, 2026.
- Where
- The results were presented at the European Society of Cardiology congress in Munich.
- Why
- The study compared the drugs’ effects on LDL cholesterol and whether patients reached their LDL-C targets.
This story does not have two clearly opposing sides.
LDL cholesterol had fallen by 54.73% with inclisiran and 17.97% with bempedoic acid.
LDL cholesterol had fallen by 56.31% with inclisiran and 15.35% with bempedoic acid.

- Participants
- 402 adults
- Study design
- First randomized, open-label phase IV direct comparison of inclisiran and bempedoic acid
- LDL-C reduction at 150 days
- 56.31% with inclisiran; 15.35% with bempedoic acid
- LDL-C target reached at 150 days
- 78.2% with inclisiran; 20.3% with bempedoic acid
- Overall adverse events
- 63.1% with inclisiran; 59.8% with bempedoic acid
What people are saying
The value of Victorion-Challenge lies in its immediate clinical relevance.
The direct comparison shows: bempedoic acid meets expectations for LDL-C lowering, but is not enough to reach recommended treatment targets in patients with ASCVD at high or very high risk.
