European groups issue guidance on two MASH treatments
Three European medical groups have issued guidance on using resmetirom and semaglutide to treat MASH, a progressive liver disease linked to metabolic problems.
The guidance says these medicines may be considered for adults with MASH and significant fibrosis but no cirrhosis.
Doctors should choose between them based on a patient’s other conditions, treatment risks and preferences; semaglutide may suit some people with obesity or type 2 diabetes, while resmetirom may be considered for some who do not use GLP-1 medicines.
The guidance says neither medicine should be used specifically to treat MASH in people with cirrhosis, because evidence on liver outcomes is not yet sufficient.
Drug treatment should accompany lifestyle changes and care for related health conditions, not replace them.
Patients should be monitored for treatment response and safety, with regular checks recommended for resmetirom and gradual dose increases for semaglutide.
Long-term liver benefits remain uncertain, and the groups do not recommend combining the medicines as first-line MASH treatment because evidence is insufficient.
The European liver, diabetes and obesity associations issued guidance on resmetirom and semaglutide for metabolic dysfunction-associated steatohepatitis (MASH).
The guidance covers patient selection, treatment initiation, monitoring and discontinuation.
It says the therapies may be considered for adults with non-cirrhotic MASH and fibrosis stages F2–F3, alongside lifestyle changes and management of metabolic risks.
The groups say neither drug should currently be used specifically as MASH-targeted therapy in patients with cirrhosis.
They do not recommend combining the drugs as first-line MASH-targeted therapy because evidence is insufficient.
